DGCA Issues Show-Cause Notice To Air India Pilot For Accepting Aircraft Despite Snags

Directorate General of Civil Aviation (DGCA) has issued a show-cause notice to an Air India pilot amid safety concerns on flights AI-358 and AI-357, pertaining to aircraft dispatch, Minimum Equipment List (MEL) compliance, and flight crew decision-making.

DGCA, in its notice, said that the pilot accepted the aircraft despite repeated snags and system degradation.

The civil aviation authority noted that there was a smell of smoke reported near a door in the AI-358 flight.

DGCA in the show cause notice said, “Whereas, it has been observed that M/s Air India Limited, during the operation of Flight AI-358 (and related operations of AI-357), serious safety concerns arose pertaining to aircraft dispatch, Minimum Equipment List (MEL) compliance, and flight crew decision-making.”

“Whereas, during the operation of Flight AI-358, the operating crew received PACK ACM L and Pack Mode advisories. And a smell of smoke was reported near the R2 door. Whereas repetitive snags related to the same systems had been recorded on five previous sectors, indicating a known history of system degradation,” the notice added.

The operating crew in AI-358, as per DGCA, accepted the aircraft without adequate understanding of MEL limitations, as on December 28, the conditions of the lower right recirculation fan were not aligned with the MEL ‘O’ conditions.

The notice stated, “Whereas, the aircraft VT-ANI was dispatched with incompatible MEL items, in contravention of CAR Section 2, Series B. Part 1. Paras 2.2 and 2.3. Whereas, despite the above, the pilots of Flights AI-358 and AI-357, including yourself, accepted the aircraft for operation with prior knowledge of repeated snags and existing system degradations.”

“Whereas, on 28.06.2025, there was non-compliance with MEL ‘O’ conditions pertaining to the lower right recirculation fan on Flight Al-358, in violation of CAR Section 2, Series B, Part 1, Para 5.3. Whereas, the operating crew accepted the aircraft without adequate understanding of MEL limitations and system interdependencies, contrary to the provisions of CAR Section 8, Series O, Part II, Para 3.1.2. Whereas the operating crew failed to adequately assess the combined operational and safety impact of multiple inoperative systems, as required under CAR Section 8, Series O, Part II, Para 4.5.1,” the aviation body said.

“Now, therefore, you are hereby directed to show cause within 14 days of receipt of this notice as to why appropriate enforcement action should not be initiated against you under the relevant provisions of the Aircraft Rules and Civil Aviation Requirements for the above-mentioned violations. Failure to submit your reply within the stipulated period shall result in the matter being decided ex parte based on the evidence available on record,” the notice read.

IED, Ammunition And Narcotics Dropped By Pakistani Drone Recovered In J&K’s Poonch

A Pakistani drone crossed into Indian territory along the Line of Control (LoC) in Jammu and Kashmir’s Poonch district and dropped suspected explosive material, ammunition and narcotics, prompting massive search operations by security forces.

According to official sources, the drone intruded into the Indian airspace in the Khadi Karmada area of Poonch and remained inside Indian territory for more than five minutes before returning across the LoC.

During the incursion, the drone dropped a consignment that included an improvised explosive device (IED), ammunition and drugs.

Soon after the drone movement was detected, the Indian Army and the Jammu and Kashmir Police launched extensive cordon and search operations in Khadi Karmada and adjoining areas to trace and secure the dropped items and to rule out any possible terrorist movement on the ground.

A video of the items dropped by the Pakistani drone has surfaced, showing the material recovered from the area.

In his statement, Dr. Rufayee emphasized that 2025 witnessed a stronger commitment by journalists and media organizations toward truth, accountability, and public interest. Despite challenges such as misinformation, commercial pressures, and safety concerns, the media largely upheld democratic values by giving voice to the marginalized, highlighting governance issues, and ensuring transparency in public life.

“The media in 2025 has not merely reported events; it has shaped public discourse with maturity and responsibility. From exposing irregularities to amplifying developmental narratives, journalism has reaffirmed its role as the fourth pillar of democracy,” Dr. Rufayee said.

He particularly appreciated the role of regional and grassroots media for bringing local issues to national attention, and the growing use of digital platforms to engage citizens in informed debate. Dr. Rufayee also acknowledged the contribution of young journalists who embraced ethical reporting while adapting to new technologies and formats.

Calling for continued vigilance, Dr. Rufayee urged media professionals to remain independent, factual, and people-centric in the coming years. He reiterated NFJM’s commitment to protecting journalistic freedom, promoting ethical standards, and strengthening the professional capacity of journalists across the country.

Dense Fog Disrupts Flight Operations at Delhi Airport; 148 Flights Cancelled

 Flight operations at the Indira Gandhi International Airport (IGIA) were severely disrupted on Wednesday due to dense fog and low visibility conditions, leading to the cancellation of at least 148 flights and delays to over 150 others.

Airport officials said that 78 arriving flights and 70 departing flights were cancelled during the day. In addition, two flights were diverted from the national capital as visibility dropped to critically low levels.

In a morning post on X, the Delhi International Airport Ltd (DIAL), which operates IGIA, said visibility conditions were gradually improving, though some flight disruptions were still expected. DIAL added that on-ground teams were working closely with all stakeholders to assist passengers and provide necessary support across all terminals.

The Ministry of Civil Aviation also issued an advisory, stating that prevailing fog conditions in parts of northern India could impact flight operations at select airports. Airlines have been instructed to strictly adhere to passenger service norms, including timely dissemination of information, assistance during delays, facilitation of rebooking or refunds where applicable, and proper handling of baggage.

IGIA, one of the busiest airports in the country, handles around 1,300 flight movements daily. Dense winter fog frequently disrupts air traffic in the region during this period, affecting both domestic and international travel.

J&K Girls Enter Quarterfinals of 17th Junior National Roll-Ball Championship

The Jammu and Kashmir Union Territory girls’ Roll-Ball team stormed into the quarterfinals of the 17th Junior National Roll-Ball Championship 2025 with a dominant 17–2 victory over Pondicherry at the MA Stadium here on Tuesday.

Earlier in the tournament, the J&K girls registered an impressive 4–1 win against Karnataka and played out a draw against Rajasthan, ensuring their place in the knockout stage with a strong overall performance.

In the boys’ category, the J&K team recorded a convincing 4–1 victory over Chandigarh. However, they faced setbacks against Gujarat, losing 4–2, and against RSSA, where they went down 8–2.

The national championship, being held across three venues in Jammu, witnessed 55 matches played in a single day. More than 600 players and officials from various states and Union Territories are participating in the event, highlighting the growing popularity of roll-ball in the country.

The matches showcased emerging talent, intense competition and high-energy performances. A large gathering of sports officials, team managers and enthusiastic spectators added to the vibrant atmosphere of the championship.

With the league stage concluded, the J&K teams will now focus on preparations for the knockout rounds, aiming to bring laurels to the Union Territory in both the girls’ and boys’ categories.

88 Injured in Loco Train Collision Inside Vishnugad–Pipalkoti Tunnel in Uttarakhand

 At least 88 people were injured after two loco trains collided inside the Pipalkoti tunnel of the under-construction Vishnugad–Pipalkoti hydropower project in Chamoli district on Tuesday night, officials said.

The accident occurred around 8.30 pm at the Tunnel Boring Machine (TBM) site inside the tunnel being constructed by THDC (India). One of the loco trains was carrying workers and officials for the night shift, while the other was transporting construction material.

According to Chamoli District Magistrate Gaurav Kumar, the train carrying workers had travelled nearly two kilometres inside the tunnel when another loco train approaching from the opposite direction lost control and rammed into it. At the time of the accident, 109 people—mostly labourers—were on board the workers’ train.

Uttarakhand Chief Minister Pushkar Singh Dhami directed authorities to ensure proper medical treatment for all the injured. The District Magistrate and Superintendent of Police Surjit Singh Panwar visited Gopeshwar District Hospital to assess the condition of those injured.

Officials said none of the injured suffered serious injuries. Seventy labourers were brought to the district hospital in Gopeshwar, where 66 were discharged after receiving first aid, while four remain under treatment. Another 18 labourers were given first aid at Vivekananda Hospital in Pipalkoti and later sent home.

Meanwhile, the Railways clarified that the trains involved in the collision had no connection with Indian Railways. In a statement, it said the incident involved a trolley-based transportation system used locally within the tunnel during the construction of the hydropower project, and not an Indian Railways train.

Officials explained that rail-like vehicles are commonly used inside long tunnels to transport workers, staff and construction material during excavation and related works.

The Vishnugad–Pipalkoti hydropower project, located between Helang and Pipalkoti on the Alaknanda River, is designed to generate 444 megawatts of electricity through four turbines and is targeted for completion next year.

2025 Focused on Governance Reforms, Ease of Living: Dr Jitendra Singh

Union Minister of State for Personnel, Public Grievances and Pensions, Dr Jitendra Singh, on Tuesday said that 2025 marked a significant phase in governance reforms, with a strong emphasis on improving ease of living for citizens and creating a more enabling, work-friendly environment for government employees.

Addressing a year-end press conference, the Minister reviewed the achievements of the Ministry of Personnel, Public Grievances and Pensions, covering the Department of Personnel and Training (DoPT), the Department of Administrative Reforms and Public Grievances (DARPG), and the Department of Pension and Pensioners’ Welfare (DoPPW).

Dr Singh said the reforms undertaken during the year were guided by the principle of “minimum government, maximum governance”, focusing on simplification of procedures, reduction of pendency and extensive use of technology to make governance more responsive and citizen-centric. He noted that governance outcomes in 2025 reflected a maturing reform ecosystem where digital tools, institutional innovation and human-centric approaches converged to deliver tangible improvements in public service delivery.

Highlighting capacity building as a key pillar, the Minister said Mission Karmayogi continued to expand rapidly. The iGOT-Karmayogi platform crossed 1.45 crore registered government employees and now offers over 4,155 e-learning courses in 23 languages, with 28 States and Union Territories onboarded. New initiatives introduced during the year included the iGOT Marketplace, specialisation programmes in seven thematic areas such as artificial intelligence, cybersecurity and public policy, and AI-enabled tools like AI Sarthi, AI Tutor and the Capacity Building Planning tool to support role-based skilling.

On transparency and citizen-centric reforms, Dr Singh said the RTI Online Portal was expanded to cover 2,899 public authorities with enhanced security features. He also highlighted improvements in service-related grievance redressal through the Central Administrative Tribunal (CAT), which achieved a disposal rate of 92.94 per cent by September 2025. The performance was supported by digital case management systems, upgraded infrastructure in Lucknow and Guwahati, and wider regional outreach through circuit sittings.

Referring to grievance redressal reforms, the Minister said the Centralised Public Grievance Redress and Monitoring System (CPGRAMS) remained a key citizen engagement platform. During 2025, over 20 lakh public grievances were disposed of, with an average resolution time of 15 days in Central Ministries and Departments. More than 10 lakh citizen feedback responses were received, alongside the introduction of new monitoring mechanisms, training modules and national workshops. He also mentioned the observance of Good Governance Week and the Prashasan Gaon Ki Ore campaign, aimed at taking administration closer to citizens across States and districts.

Dr Singh said Special Campaign 5.0, implemented across 84 Ministries and Departments, focused on institutionalising cleanliness and reducing pendency. The campaign covered 11.6 lakh cleanliness sites, resulted in the weeding out or closure of 29.52 lakh files, freed 233.75 lakh square feet of office space and generated Rs 833.92 crore through scrap disposal, contributing to improved efficiency and workplace practices in government offices.

Earlier, DoPT Secretary Rachna Shah outlined the Ministry’s priorities, stating that all reforms during the year were anchored in transparency, efficiency and citizen-centricity.

The press conference was attended by Manoj Kumar Dwivedi, Additional Secretary, DoPT; Puneet Yadav, Additional Secretary, DARPG; and Dhrubajyoti Sengupta, Joint Secretary, DoPPW, reflecting coordinated efforts across the three departments under the Ministry.

39 OGWs Identified In J&K’s Kathua This Year, 18 Of Them Booked Under PSA

Police identified 39 overground workers (OGWs) and booked 18 of them under the Public Safety Act in Jammu and Kashmir’s Kathua district during 2025, marking a major crackdown on terror networks in the region, a senior officer said on Wednesday.

Presenting the annual crime report for 2025, Kathua Senior Superintendent of Police (SSP) Mohita Sharma said four militancy-related cases were registered during the year, leading to the arrest of seven persons, while two terrorists were eliminated.

“A major crackdown was launched against terrorism-linked networks in Kathua district. We identified 39 OGWs and booked 18 of them under the Public Safety Act during 2025,” Sharma said.

She said a significant breakthrough came in a drone-dropping case registered under the Narcotic Drugs and Psychotropic Substances (NDPS) Act, in which 447 grams of contraband substance were seized, with the arrests of eight persons so far.

On cybercrime, the SSP said Kathua Police recovered nearly Rs 4 crore, which was handed over to the complainants.

Additionally, during floods in the district, 263 people were rescued by Kathua Police with assistance from other agencies, she said.

Highlighting future priorities, Sharma said 2026 would be observed as the “year of technology” for the police, with greater reliance on high-tech systems and artificial intelligence to improve crime detection and investigation.

The SSP also said an alert has been sounded on December 31 to ensure safety and security during New Year celebrations, urging people to maintain peace and order. (Agencies)

Rajnath Hoists Flag In Ram Temple Complex To Mark Second Year Of Idol Consecration

Defence Minister Rajnath Singh on Wednesday hoisted a flag at the Annapurna Temple in the Ram Temple complex and joined prayers on the occasion of the second anniversary of Pran Pratishtha.

The veteran BJP leader was accompanied by Uttar Pradesh Chief Minister Yogi Adityanath.

The Wednesday event followed a programme on November 25, when Prime Minister Narendra Modi hoisted a saffron flag atop the Ram Temple, marking the formal completion of its construction.

Before reaching the Ram Temple complex, Singh and Adityanath prayed at the Hanumangarhi Temple here.

Singh also prayed before the idol of Ram Lalla and participated as the chief yajman at the Pran Pratishtha Dwadashi programme, officials said.

The Pran Pratishtha Dwadashi rituals are scheduled to continue till Friday.

Religious rituals at the temple began on Saturday.

An idol of Ram Lalla was consecrated at the temple on January 22, 2024, during a ceremony led by Prime Minister Narendra Modi.

A Shri Ram Janmabhoomi Teertha Kshetra Trust office-bearer said an estimated five to six lakh devotees are expected to come to Ayodhya for Ram Lalla’s darshan for the second anniversary. (PTI)
